Multiple precautions are in place to prevent switching me to a ventilator that is not ventilating. Yet it happened again.
1. "not ventilating" alarm - all alarms had been muted
2. check that the rubber lung is expanding - "I thought it was"
3. ask me if it is ok to do the change - you should also pay attention to my reply (my eyes were wide open and indicating NOOO!!)
4. check that my chest is rising before turning away - "nobody has told me to do that"
Hands-on experience is why I have zero confidence in human operations, no matter what the grace period.
