It's possible because of a partnership between Trenitalia, Deutsche Bahn and Österreichische Bundesbahnen (because it has to pass through Austria). Milan <-> Munich ~6:30h Rome <-> Munich ~8:30h Future plans include extensions to Berlin and Naples by 2028, and there are discussions for Italy to Vienna too, but nothing concrete yet.
